Fresh Start of California LLC is committed to maintaining the privacy and security of your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and all applicable state and federal confidentiality laws.
Use and Disclosure of Protected Health Information (PHI)

Safeguards and Security
All Fresh Start staff members are trained in HIPAA compliance and sign confidentiality agreements upon hiring. Clinical records are securely stored in locked physical files and encrypted electronic systems. Access is limited to authorized personnel only.
Fresh Start uses HIPAA-compliant software platforms for medical records, telehealth services, and insurance claims. Any external partners or vendors (e.g., billing services or cloud storage providers) are required to sign Business Associate Agreements (BAAs) ensuring they also follow HIPAA guidelines.

Federal and state law permits certain disclosures without client authorization. These include:

Violations and Breaches
